A REPORT by a coroner, sickened by cases where failures of the health system resulted in a patient's death, has finally spurred the Premier to announce a special commission of inquiry.

The Deputy State Coroner, Carl Milovanovich, found "systemic problems" were to blame for the death of a schoolgirl at Royal North Shore Hospital.

Vanessa Anderson, 16, had been loaded with an inappropriate amount of opiates which had depressed her breathing, resulting in respiratory failure.
