| THE state's failure to provide an effective response to domestic violence has contributed to 36 deaths on average a year, police repeatedly being called out to the same families, and a $4.5 billion annual loss in economic activity.
A scathing report by the NSW Auditor-General on the response to domestic violence has found most victims still do not report the incidents to police, and those that do are unlikely to get a lasting solution because of an unco-ordinated systems response.
''My concern is that the response before and after crises is fragmented,'' the Auditor-General, Peter Achterstraat, said yesterday.
