Jonathon Craig Candy collapsed at the Marion Shopping Centre in Adelaide's south in May 2010.
A member of the public tried to resuscitate him before paramedics arrived eight minutes later.
Mr Candy was pronounced dead after he was taken to the Flinders Medical Centre.
The 42-year-old had a history of heart disease and was admitted to the Queen Elizabeth Hospital a day earlier after suffering chest pains at work.
The coroner found that hospital staff did not obtain a proper history of Mr Candy's condition, with nurses only noting chest pains he had felt the night before.
